SECHD Home Health and Hospice Agency serves all of Baca County. We offer services to Medicare, Medicaid, Commercial insurances, and private pay patients.
What is Home Health:
Our team provides a wide range of health care services throughout Baca County and the Kim area. Our team consists of providers, nurses, therapist, and social workers who committed to providing high quality health care for patients and their families in the home setting. Home health care includes skilled nursing care as well as other skilled care services such as physical and occupational therapy, speech therapy, and medical social services.
Our goal is to help coordinate the care and/or therapy your medical provider has ordered. The patient and team will work together to provide an individualized plan of care that will be used to help the patient reach and keep their best physical, mental, and social well-being.
Services we offer include management of acute and chronic disease; medication management; wound care management including the use of wound vacs; lab draws; coumadin therapy; diabetes management; in-home physical, occupational, and speech therapy; social and psychosocial care; patient and family education; and care giver training.

What is Hospice:
We offer a special healthcare option for patients who are faced with terminal illness, and their families. Our multi-disciplinary team consists of providers, nurses, therapists, social workers, clergy, bereavement counsellors, and volunteers. Our team is committed to providing physical, social, emotional, and spiritual needs to each patient and their family. Here at Southeast Colorado Hospital District Home Health and Hospice we value our patients' wishes. We pride ourselves in being able to provide end of life care for patients and their families who wish to remain at home during the last stage of their life.Our goal of hospice care isn't to cure underlying disease, but to provide patient dignity and the highest quality of life possible through compassionate care for the patient and their families, pain and symptom management, counseling, end of life education, and caregiver support, education, and training.
Cancer Travel Fund:
Our travel fund is available to anyone in Baca County who has been diagnosed with cancer. Funds are available to help assist with travel related expenses while the patient is receiving treatment.
