What is Long Term Medicaid Home Health?

We provide long-term care services at home for eligible individuals. Skilled Nursing care can provide services like wound care, medication administration and management, and patient education on chronic illnesses.

What is Acute Home Health Care?

Acute Home Health care refers to short-term, intermittent skilled services provided in the patient’s home, typically for a period of up to 60 days, following a hospitalization or acute onset of an illness, injury, or disability. For all Medicare patients, you must be homebound.

We accept Medicare Medicaid, eligible commercial insurances, and eligible VA insurance.

Our focus is adults who are 18 years and older. We serve the BACA County area and the town of Kim, CO.
