What is Hospice?
A specialized form of healthcare focused on providing comfort support to the terminally ill patients and their families. We focus on shifting the focus of care from curative to managing pain, symptoms, emotional, physical needs, and improve quality of life during the patient’s last journey. We offer regular visits by the RN with specialized training and experience on pain and symptom management. Also involves coordinated team of professionals who offer medical, emotional, and spiritual support. We focus on providing care in the privacy of the patients home with their loved ones at their side and we help educate the family on care on the dying process. Also help families make necessary transitions. We focus on end of life care with adults
Medical Prognosis: A patient is typically eligible for hospice if terminal illness with prognosis of less than 6 months or less to live if illness runs its natural course.

Volunteers: Provide companionship, assistance, and support throughout the end of life care process.
Spiritual: Spiritual care and support as requested by patients and their families.
We serve the Baca County area along with the town of Kim Colorado.
Hospice is fully funded by Medicare.

Donations
Memorial: Can make donations in memory of a loved one or can be made directly to Baca County Hospice at any time.
Tree of Lights: Donations made through our yearly fundraiser. An ornament can be purchased in the memory of a loved one and placed on the “Tree of Lights” These funds are used to help with the cost of education, training, and supplies.
Cancer Travel Fund: Donations from this fund help Baca County patients that are going through treatment and need assistance with travel supplement. We work with Cancer League of Colorado, Salvation Army, and private donations.
Cancer Travel Fund
Our travel fund is available to anyone in Baca County who has been diagnosed with cancer. Funds are available to help assist with travel related expenses while the patient is receiving treatment.
